I NEED TO RENEW MY BADGE

It is the responsibility of each badge holder to maintain a current airport-issued identification badge.

Your badge must be renewed before the badge expires. All badge renewals require an appointment and badges can be renewed up to 30 days before expiration. If your badge becomes expired for more than 30 days, you will have to go through the entire badging process again as a new applicant. For your badge renewal appointment, please bring with you the following completed forms:

1. Badge and Fingerprint / STA Application (This form will not be accepted unless it is signed by your Authorized Signer.)
2. Privacy Act Notice

You will also need to provide the Badging Office your two forms of ID from the List of Acceptable Documents. Copies are not accepted. For examples of acceptable forms of identification, please visit the US Citizenship and Immigration Services website www.uscis.gov.

The Badge and Fingerprint / STA Application must be completed in its entirety. If you meet one of the following three conditions listed below, you must provide those identifications used to complete the Badge and Fingerprint / STA Application. The three conditions are:

1. You are not a U.S. citizen
2. You hold a non-immigrant Visa
3. You are a U.S. citizen born abroad or a naturalized U.S. citizen

For SIDA badges (blue), AOA badges (green) and anyone who drives, security training is taken via the IET (Interactive Employee Training) computer or a via classroom instruction provided by LCH. There are security, Non-Movement Area Driving and Movement Area Driving components. Please allow about one hour for security training and 30 minutes for each driving training component. In addition, depending on your particular access level, there may be an additional security brief to read and initial. Further, for those individuals with a Public ID badge, a security awareness training module will also be completed on the IET computer or through classroom instruction. Once your training or security briefs are successfully completed, the new badge is issued.

All identification badges issued by LCH are the property of the Airport and must be returned immediately under the following conditions:

1. Upon expiration of the badge (it is a serious security violation to display an expired badge)
2. Upon separation of employment (for any reason)
3. When job function no longer requires an airport-issued identification badge
4. Upon demand of the Lake Charles Regional Airport
5. Upon conviction of any of the disqualifying crimes (see CFR Title 49: Transportation § 1542.209 letter (d) for a list of disqualifying crimes).

All badges that are lost, stolen or otherwise unaccounted for must be reported immediately to the LCH Administration office at (337) 477-6051 x. 200.

I NEED TO APPLY FOR A BADGE

Applying for a new badge is a two-appointment process with the LCH Department of Public Safety. For your first appointment, please bring with you the following completed forms:

1. Badge and Fingerprint / STA Application (This form will not be accepted unless it is signed by your Authorized Signer.)

All badge applicants will be fingerprinted unless:

a. You are an airline employee and were fingerprinted as a condition of your employment and you have not had a break in service of more than 30 days. Please bring with you a completed CHRC Form for Airlines or provide the Badging Office with a Certification of Criminal History Investigation / Continual Employment on company letterhead.
b. You are applying for an AOA badge or ID card.
c. You are an employee of the Federal, state or local government (including a law enforcement officer) who, as a condition of employment, has been subjected to an employment investigation that includes a criminal records check.

The paperwork and fingerprinting process constitute the first appointment. The CHRC (Criminal History Records Check) and STA (Security Threat Assessment) clearances must occur before the individual proceeds any further in the process. Please note these checks and clearances can take up to two weeks.

Seven to ten (7 – 10) business days after your initial appointment, please contact the Administration Office at (337) 477-6051 Ext. 200 to inquire about your clearances. If all required information has been received, your second appointment will be made for you to take your training. MY COMPANY NEEDS AUTHORIZATION TO OBTAIN BADGES

The following information will introduce your company to the badging procedures at the Lake Charles Regional Airport. To begin, if your company does not have a current agreement with the Airport, please contact the Airport at 337-477-6051 ext. 200 to have your “Authorization to Badge form” completed. Your Airport contact person will obtain all the necessary information from you and the completed form will be submitted to Airport Operations. This form, which is signed-off by the Airport Security Coordinator, outlines your company’s access level, driving privileges (if necessary) and indicates your company’s security training requirements.

Next, the Authorized Signature Letter form must be completed. This form includes one Primary Authorized Signer and one Secondary Authorized Signer who are authorized to sign the Badge and Fingerprint / STA Application. The Badging Office will only accept this form with one or two signatories (and it is highly recommended to have two Authorized Signers for companies with more than ten employees). Additional signatories are not permitted. Additionally, this form requires both a printed name and a signature on the form. All employees listed on the Authorized Signature Letter form must obtain an airport-issued identification badge (see “Badges”) by completing a Criminal History Record Check (CHRC), Security Threat Assessment (STA), security training and the Authorized Signer training.

The Authorized Signer training for your Authorized Signer(s) consists of a module on the IET (Interactive Employee Training) computer and the “Authorized Signer Training form.” The computer training and the form must be completed in the Airport Badging Office by the Primary and Secondary Authorized Signers. Please allow at least 30 minutes for this training. The Authorized Signer Training will communicate the signatory’s responsibilities and duties and requires all signatories to certify they have completed and understand the training. The Authorized Signer Training is required annually. Contact the Administration Office at (337) 477-6051 Ext. 200 to make an appointment to complete your AS training.

MY COMPANY HAS A NEW AUTHORIZED SIGNER(S)

Please complete a new Authorized Signature Letter form and return the original to the Badging Office. In addition, all Authorized Signers are required to complete annual training. This training must be completed in the Badging Office and consists of an Authorized Signer training module taken on the IET (Interactive Employee Training) computer and the “Authorized Signer Training form.” Please allow at least 30 minutes for this training. Contact the Administration Office at (337) 477-6051 Ext. 200 to make an appointment to complete your training.

I HAVE AN EMPLOYEE WHO IS NO LONGER WITH MY COMPANY

If you have not done so already, call the LCH Administration office at (337) 477-6051 x. 200. to make your notification. In addition, please fill out the Lost/Stolen Badge or Key form and return it to the Badging Office.

I NEED TO PROVIDE VERIFICATION OF THE BADGE AND KEY AUDIT

Audits are required by the Transportation Security Administration (CFR 49 Part 1542). Audit forms are distributed in hard-copy form by the Airport Security Coordinator to the Authorized Signers. The Authorized Signer will be responsible for completing the audit and providing the following information for all listed employees:

Employment status (badge and key must be returned if an individual is no longer employed. Also complete the Termination Notification form
Location of badge (retained by employee, returned to company, lost, etc.)
Verification of key(s) held by each badged employee
